Patient Forms

Getting your forms right before a dental visit saves time and helps the team prepare properly. Use this guide to see what to bring, what to complete, and when to send records through.

Patient Forms

Start with the forms we need for your appointment

Before you arrive at Techno-Dent Clinic in Sandton, please complete the patient forms relevant to your visit. This gives our team the details needed for new patient consults, emergency dentistry, family dentistry, cosmetic consulting, implant planning, orthodontic assessments, sedation options, aftercare follow-ups and payment guidance.

If you are coming in from Bryanston, Rivonia, Morningside, Fourways or Rosebank, it helps to fill in the paperwork at home so your appointment can start on time. It also gives us time to check any medical risks, previous treatment notes and current concerns before we see you.

What to bring on the day

Please bring the basics with you, even if you have already sent some details ahead. We use these items to confirm your recrods, update your file and make sure your treatment plan is accurate.

  • Your ID document or passport
  • Your medical aid card, if you have one
  • A list of current medication and dosages
  • Details of allergies, especially to medicine or latex
  • Any previous dental records, X-rays or scans
  • Contact details for your medical aid or payment authorisation, if needed

How to send records before your appointment

You can email supporting records through our contact form before your visit. If you have referral notes, old X-rays, digital scans, photographs or treatment summaries from another practice, send them through as attachments where possible. Clear files help Dr Sibusiso Mthembu and the relevant clinician review your case properly before you sit down.

For larger files, name them clearly with your surname and the date. If something is missing, our practice manager can advise what still needs to be sent. Please do not wait until the last minute if your appointment is for implants, orthodontics or cosmetic planning, because those visits often depend on background records.

Medical details that help us plan safely

Dental care is not just about teeth. We need a complete picture of your health so the team can plan treatment safely, especially if you are booking sedation options, surgery, or complex restorative work. This is standard practice in South Africa and it helps us avoid delays on the day.

  • Heart conditions, diabetes, epilepsy or asthma
  • Pregnancy or recent surgery
  • Blood thinners or long term medication
  • Previous reactions to local anaesthetic or antibiotics
  • Bleeding problems or implant history
  • Anxiety around treatment, so we can allow extra time

For parents and caregivers bringing children

Family dentistry visits work best when a parent or legal guardian helps complete the paperwork. If you are bringing a child for preventive care or an orthodontic assessment, please make sure the consent section is signed by the correct adult. Where a child has previous treatment notes, older X-rays or a referral letter, send those ahead as well.

Children often feel more settled when the first few minutes are organised. A completed form, a clear contact number and a good list of medical details make the visit easier for everyone, and it keeps things moving if the child needs follow-up care.

If you need help with payment and admin

We know that dental treatment can involve planning, especially where implants, cosmetic work or orthodontics are concerned. If you need payment guidance, please tell us early so we can explain the usual steps and what documents may be needed for pre-authorisation or quotations. Lerato Moloi and the front office team can help with practical admin questions before your appointment.

If you are not sure which forms apply to your visit, just ask. It is better to confirm in advance than to arrive with missing information and have to reschedule. That is especially useful for patients travelling in from across Sandton Johannesburg during busy traffic or load shedding hours.

Questions we get asked

Do I need to complete patient forms before every visit?

New patients should complete the full set of forms, and returning patients may need an update if their medical details, medication or contact information has changed.

Can I email my X-rays or previous dental records?

Yes. Send relevant records through our contact form before your appointment so the clinical team can review them in advance.

What if I do not have medical aid or all my paperwork with me?

Please still attend if you can. Bring whatever records you have and let us know what is missing so we can advise on the next step.
